Describe the service workload

Record the scheduled assessment or education periods and any additional coverage requested. Note time allocated for reviewing information and completing documentation. Keep group education work distinct from individual service assignments when comparing expectations.

Separate workplace direction from care

If feedback concerns a plan or service, record who provided it and whether it addressed timing, documentation or clinical judgment. Describe any request for clarification without reproducing patient details. Do not treat a professional disagreement as proof of an employment motive.

Protect nutrition records

Do not retain laboratory results, dietary histories, meal plans or client-identifying education materials. Use your own employment records and permitted management messages. Identify the official record location and date range if an authorized reviewer needs further context.

Choose your next step

Review the workplace retaliation guide, unpaid wages guide, or discrimination guide if those concerns fit your experience. A job title alone does not establish a claim.
