Identify the task and supervision

Describe the procedure or administrative responsibility in general terms, the training you had received and who directed the work. Note changes in assignment or requests for help. Avoid guessing whether an instruction was clinically appropriate; identify what was requested and what you observed so the employment issue can be reviewed separately.

Track room preparation and workflow changes

Record scheduled hours, preparation duties, cleanup and any movement between clinical and front-office responsibilities. If an evaluation cited delays, note the equipment, supplies or approval needed at the time. Match relevant events to your own evaluation or permitted manager correspondence. Do not use patient appointments as a substitute for a personal work-time record.

Keep clinical materials out of personal storage

Do not copy charts, laboratory labels, appointment lists, specimen photographs or patient messages. A personal timeline should omit names and details that could identify a patient. Identify the type of institutional record that exists if needed for review, and seek handling guidance before sharing any clinical information. Keep personal employment and training documents separate.

Choose your next step

Review the workplace retaliation guide, unpaid wages guide, or discrimination guide if those concerns fit your experience. A job title alone does not establish a claim.
