“Not published” preserves unavailable or suppressed source cells. “Above reporting limit” preserves a top-coded source value without inventing a number. A missing state estimate does not establish service eligibility. The data describe wage and salary employment; they do not measure self-employment income.

Clarify the assigned activity
Describe the activity or adaptive-equipment instruction generally and the supervising role. Note a change in the plan or a request for assistance, including the response. Distinguish your implementation task from decisions about treatment goals or scope, which this record guide does not determine.
Explain preparation and reporting
List setup, activity support, documentation and coordination relevant to your schedule. Compare your personal time entry with those duties, identifying estimates. A client’s session record may not capture all preparation and should not be copied to reconstruct your workday.
Protect rehabilitation information
Do not retain progress notes, patient images, home-program materials or family communications in personal storage. Identify record types for controlled review. Keep your own training, job description and employment feedback separately, without making clinical claims about why a treatment result occurred.
Choose your next step
Review the workplace retaliation guide, unpaid wages guide, or discrimination guide if those concerns fit your experience. A job title alone does not establish a claim.
