A missing state estimate does not establish service eligibility. The data describe wage and salary employment; they do not measure self-employment income.

Identify the plan and assignment generally

Describe the activity and the role responsible for directing or approving changes. Note when you requested clarification or assistance and what response followed. Separate your observation from a conclusion later supplied by a therapist, and omit identifying details about the person receiving care.

Explain work surrounding treatment

Record preparation, coordination and documentation duties relevant to your own schedule or evaluation. If feedback cites delay or output, identify the standard communicated to you. Appointment or progress-note timestamps may not show every assigned activity or establish your complete work period.

Protect clinical documentation

Do not copy progress notes, treatment plans, patient images or home-exercise materials into personal storage. Identify relevant record categories for appropriate review. Keep your own permitted training, employment correspondence and evaluation separate from patient records, and avoid accessing a chart solely for an employment dispute.

Choose your next step

Review the workplace retaliation guide, unpaid wages guide, or discrimination guide if those concerns fit your experience. A job title alone does not establish a claim.
